Board forms are now provided in a fillable format on our website.  You can also download the forms to your storage media.  If you have any questions, please call the Board’s Call Center at (404) 656-3818 or 1-800-533-0682.

The Georgia State Board of Workers’ Compensation provides all forms, upon request, free of charge. To request paper copies of forms, please call (404) 656-3870.

Do not send any additional copies of any forms when filing in paper. Stamped copies will not be returned.  A Stipulated Settlement is the only exception to this rule. Please call (404) 656-2929 if you have any questions about a Stipulated Settlement filed in paper.
