Conservator Criminal History Requests

The Enforcement Division is now able to run multi-state criminal history background checks for Board Form 226a Petition for Appointment of Temporary Conservator of Minor(s) and Board Form 226B Petition for Appointment of Temporary Conservator of Legally Incapacitated Adult. No longer will a petitioner who has lived outside of Georgia be required to obtain a criminal history from that state. Please note however, the Enforcement Division cannot run a criminal history on persons living outside the United States.

The petitioner must still completely fill out Board Form 226B to include the "Consent Form". This portion must be signed by the petitioner and notarized.
