Board Form Policy

Effective April 15, 2006, the State Board will only accept forms with the current revision date.  Outdated forms will be returned to the sender.  Board Forms are revised in July of each year.  Please go to (https://sbwc.georgia.gov/forms/board-forms) and click on publication and forms.

 Effective July 1, 2023, the Board forms are available in the fillable format for downloading to your storage media.
