Summary of 2009 Forms Changes

There were 10 form changes this year.
 
WC-1 - There were 2 changes in Section A - "Guarantee Fund" to "Group Fund" - "Date Employer had knowledge of Initial Disability" to "Date Employer had knowledge of Injury".
 
WC-2 - There were 2 changes in Section C - number 4 changed to "Rule 221(i) (4)" and number 10 changed to "File the Form WC-3 simultaneously and send a copy to the employee".
 
WC-6 - We added the "off days" to this form because EDI does not have this information on the FROI (WC-1) when it is filed. 
 
WC-15 - There were 2 changes on this form - we added "and Board Rule 15" and "in compliance with and".
 
WC-26 - There were 3 changed on this form - we added "Total amount of Temporary Total Benefits paid this year", "Total amount of Temporary Partial Benefits paid this year", and "Total amount of Permanent Partial Benefits paid this year".
 
WC-104 - We changed the last sentence of the instructions to "This form, along with attached medical report, should only be filed with the Board as an attachment to a Form 2 when converting benefits from TTD to TPD".
 
WC-131 - This form had a few changes.
 
WC-207 - The wording in the body of the form was changed. 
 
WC-240 - We changed the last sentence of the instructions to "This form, along with attachments, should only be filed with the Board as an attachment to a Form 2".
 
WC-Subpoena - We added "WC-Subpoena" to the bottom of the form on both pages.
